Carease Health in Irondequoit, New York is seeking a Geriatric Nurse Practitioner (NP) to join our team. You will provide high-quality, patient-centered care to our geriatric patients, performing exams, orders, and treatment planning within RPM, CCM, and CoCM to support proactive health management.

Responsibilities

• Provide primary and specialty care to geriatric patients, including physical exams, diagnostic testing, and treatment planning
• Conduct patient assessments, develop personalized care plans, and implement evidence-based treatments
• Collaborate with healthcare teams, including physicians, social workers, and other healthcare professionals to ensure comprehensive care
• Utilize RPM, CCM, and CoCM to monitor patient health and detect abnormalities swiftly
• Develop and maintain strong relationships with patients, families, and caregivers to ensure exceptional patient satisfaction
• Participate in ongoing quality improvement initiatives to enhance patient outcomes and reduce hospitalizations
