Job summary

UnitedHealth Group in Georgia is seeking a Nurse Practitioner to support the SNF Patient Connect program across Dougherty, Worth, Benn Hill, Colquitt and Irwin counties. You will perform one-time comprehensive health assessments in collaboration with the case management team to improve care coordination and outcomes.

The role emphasizes autonomy, collaboration with primary care physicians and nursing staff, accurate documentation and coding, and closing care gaps.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the state you will be practicing in.
• Active and unrestricted driver’s license with access to reliable transportation.
• Ability to gain a collaborative practice agreement, if applicable in your state.
• Ability to navigate varied environments and to position oneself as needed to perform job duties.

Responsibilities

• Work with primary care physicians to provide the best care possible.
• Collaborate with the nursing staff and the patients' families.
• Collaboration with case management team.
