Job summary

Palm Primary Care in Texas is seeking a skilled APRN to work in our value-based primary care setting, serving Medicare Advantage, Medicare, and Medicaid populations.

The role emphasizes chronic disease management, documentation and patient relationships, with an active Texas APRN license and DEA eligible. In-person work, regular weekday schedule, and a comprehensive benefits package are provided.

Qualifications

• Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive TexasAPRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
