Palm Primary Care in Texas is seeking an experienced APRN for senior primary care in a value-based setting. Our centers emphasize compassionate, high-quality care with Medicare Advantage and Medicaid populations, backed by an 98% patient satisfaction rate.

We offer a Monday–Friday schedule with no on-call, competitive benefits including 401(k), health, dental, life, PTO, relocation assistance, and a commitment to diversity. Texas APRN license and DEA eligibility are required.

Qualificações

• Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive Texas APRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
