Qualifications

• Master's degree in nursing from an accredited school.

Responsibilities

• Provides comprehensive evaluation and management of acute, episodic, and chronic health problems for patients in skilled nursing facility and residential care stays.
• Collaborates with attending physician and provides mid-level practitioner services, including initial evaluation, ongoing medical monitoring, triage of changes in patient condition for the MD/ARNP team, and ensures appropriate medical discharge planning.
• Attends staff meetings and participates in after-hours call coverage rotation; contributes to Nursing Home Services quality improvement projects.
