Job summary

Kaiser Permanente is seeking an ARNP to provide geriatric care within Nursing Home Services in the South Pod, primarily in Olympia/Lacey. The role involves evaluating and managing acute and chronic conditions in SNF stays and residential care, collaborating with MDs and a multidisciplinary team.

Required are WA ARNP prescriptive authority, WA NP licensure, and strong geriatric experience; occasional care setting float and after-hours coverage may occur as census shifts require.

Qualifications

• Two (2) years of geriatric clinical experience in acute and chronic health problems.
• Current KP Coalition employees have this experience requirement waived as per the National Agreement.

Responsibilities

• Provides comprehensive evaluation and management of acute, episodic, and chronic health problems for patients in skilled nursing facility and residential care stays, coordinating with the attending physician and ARNP team.
• Monitors quality of care, educates patients and families about medical treatment options, and participates in care conferences and discharge planning.
• Ensures accurate documentation, participates in after-hours call rotation, and supports quality improvement projects within Nursing Home Services.
