This role requires MSN/DNP or NP certificate, national certification (AANP/ANCC), an active DEA license, and 2+ years in geriatric or family

Qualifications

• MSN/DNP or NP certificate program completed.
• National certification (AANP or ANCC).
• Active DEA license.
• 2+ years in geriatric, adult, or family practice.
• Passionate about healthcare for seniors and low-income populations.

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions in residents.
• Develop treatment plans and communicate them to residents and other providers.
• Order and interpret labs and diagnostic tests; adjust treatments as needed.
• Maintain compliant documentation in the EMR system.
• Collaborate with PCPs and interdisciplinary teams to meet goals.
• Coordinate care with telehealth and other providers to ensure continuity.
• Provide a positive patient experience and focus on resident satisfaction.
