Job summary

Personalized Health Partners seeks a Nurse Practitioner (Adult/Gerontology) for Holbrook Healthcare Center in Buckhannon, WV. This full-time role is weekdays with no on-call, weekends, or holiday coverage, working in a collaborative team to deliver high quality primary care to seniors.

Responsibilities include diagnosing and managing conditions, patient education, treatment planning, and coordinating care with PCPs and telemedicine providers. Competitive salary with quarterly bonuses.

Qualifications

• MS in Nursing or DNP/NP certificate required.
• National certification (AANP or ANCC) active.
• Active, unrestricted state NP license.
• 2+ years clinical experience in geriatric/adult/family settings.
• US work authorization.
• Ability to work in a flexible environment.

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions.
• Provide health education and develop treatment plans.
• Order and interpret lab/diagnostic tests and adjust plans.
• Maintain documentation in EMR and coordinate with care team.
• Collaborate with PCP, telemedicine, and interdisciplinary teams.
• Participate in family conferences and advanced care planning.
