Job summary

CommuniCare Family of Companies seeks a Nurse Practitioner to provide high-quality primary care to residents at Mount Vernon Healthcare Center in Alexandria, VA. You will diagnose, treat, and manage acute and chronic conditions in a collaborative team model, focusing on person-centered care and reducing hospital visits.

The role operates Monday–Friday, 8a–5p, with no on-call, weekends, or holidays. Requires NP certification, VA licensure, and 2+ years in geriatric/adult care, plus strong

Qualifications

• Master of Science in Nursing or Doctoral/APRN terminal degree.
• National certification (AANP or ANCC) active.
• Unrestricted state license and active DEA registration.
• 2+ years geriatric/adult/family practice NP experience.
• Strong communication and team collaboration skills.

Responsibilities

• Assess, diagnose, and manage chronic and acute conditions in seniors.
• Develop and discuss treatment plans with residents and care teams.
• Order/interpret labs and imaging; adjust care as needed.
• Document accurately in the EMR and ensure compliance.
• Collaborate with PCP and interdisciplinary team to optimize care.
