Job summary

PACE SEMI in Southfield, MI is seeking a part‑time Nurse Practitioner to provide primary care for an assigned panel of participants. You will manage acute and chronic care under the supervision of the center Physician‑In‑Charge, primarily in clinic with some home visits and telemedicine.

Responsibilities include chronic disease management with quarterly assessments, medication management including controlled substances, and care coordination with the interdisciplinary team.

Qualifications

• RN with 3–5 years clinical experience, geriatrics preferred
• Must hold active Michigan NP license and CME requirements must be met
• TB screenings, infection control, BLS and in‑service requirements are part of duties

Responsibilities

• Chronic Disease Management: monthly comprehensive annual and semi-annual assessments
• Chronic Care Management visits every 3 months
• Pharmacy prescribing including controlled substances with supervising physician delegation
• Specialty and diagnostic referrals with physician guidance
• Documentation in SOAP format within 24–72 hours
• Utilization Management contributions
• Facilitate Goals of Care and end‑of‑life discussions with participants and families
• Care coordination with IDT, PCT, UM, and other PACE services
• Location of care includes clinic, participants' homes, nursing facilities and telemedicine
• After-hours on-call medical assistance on a rotating basis as needed
