PruittHealth Premier Services seeks a Nurse Practitioner or Physician Assistant to collaborate with physicians and RNs to deliver high-touch, compassionate care to residents, coordinating care and managing complex cases in long-term care settings.

Requirements include graduate NP (Adult/Family) or PA studies, GA licensure, 2 years in geriatric/adult/family practice, board certification (AANP/ANCC), active NPI and Medicare numbers, and willingness to travel between care sites.

Qualifikationen

• Graduate of an accredited Adult or Family NP program or PA Studies.
• GA RN/NP or PA licensure; board certification (AANP/ANCC) in Adult/Family/Geriatrics.
• Active National Provider Number (NPI) and Medicare provider number.
• Minimum 2 years in geriatric, adult or family practice settings.
• CPR certification and ongoing licensure compliance.

Aufgaben

• Collaborate with physicians, case managers and staff to coordinate patient care.
• Perform comprehensive adult/geriatric assessment and history.
• Document diagnoses and care plans; monitor clinical progress.
• Reconcile medications and evaluate treatment effectiveness.
• Participate in Quality Assurance and care transition processes.
• Provide education and support to residents and families.
• Travel between care sites as required; participate in on-call coverage.
