Job summary

PruittHealth Premier Services seeks a Nurse Practitioner or Physician Assistant to deliver high-touch, patient-centered care in long-term care settings. You will collaborate with physicians, RNs, and care teams to assess, diagnose, and manage acute and chronic conditions while coordinating transitions between care levels.

The role emphasizes care coordination, documentation accuracy, and participation in quality improvement activities.

Qualifications

• Graduate of an accredited Adult or Family NP or PA program.
• 2+ years clinical experience in geriatric, adult or family practice.
• Active RN/NP/PA licensure with board certification and CPR certification.

Responsibilities

• Perform comprehensive adult/geriatric history and exams.
• Identify, diagnose and document acute and chronic diseases in adults.
• Coordinate care with physicians, case managers, social workers, and families.
• Pursue ongoing education and professional development.
• Reconcile medications and review orders for appropriateness.
• Document needs of clinically complex patients and communicate care plans.
