Job summary

PruittHealth Premier Services is seeking a Nurse Practitioner or Physician Assistant to join our geriatrics team. You will collaborate with physicians, nurses, and care managers to deliver high-touch, coordinated care in long-term care settings.

This role requires state licensure (GA preferred) with NP/PA certification, active NPI, and Medicare provider numbers. On-call pay and flexible scheduling are offered, with a strong emphasis on compassionate, patient- and family-centered care.

Qualifications

• Graduate of an accredited Adult or Family Nurse Practitioner Master’s Degree program or Physician Assistant Studies.
• Current and unrestricted RN and NP licensure, or PA licensure in state of practice.
• Board certification by AANP or ANCC in Adult, Family Practice and Geriatrics or Adult-Gerontology Primary Care.
• Current CPR certification.”
• Must have an active National Provider Number (NPI) and Medicare provider number.

Responsibilities

• Perform comprehensive adult/geriatric admissions history and physical exams.
• Identify, diagnose, and document episodic, acute and chronic disease in adults.
• Collaborate with physicians, case managers, social workers, and families.
• Coordinate care transitions across levels of care and settings, ensuring timely ICP communication.
• Maintain EMR documentation and ICD-10 coding accuracy.
• Participate in Quality Assurance and Performance Improvement activities.
• Reconcile medications and optimize pharmacotherapy in collaboration with clinicians.
• Provide care planning and education for residents and families.
