Telos Health Systems seeks an experienced Advanced Practice Provider (NP/PA) to serve as the in-house SNF-ist for geriatric residents in Waverly, TN, providing comprehensive internal medicine care with autonomy and collaboration with the care team.

The role requires a current NP or PA-C license, DEA registration, and a Master’s degree. Expect flexible pay-per-encounter, W-2 employment, and a supportive leadership structure to aid long-term resident care.

Qualifications

• Active, unrestricted APRN/APN or PA-C license in the state of practice.
• Current DEA registration.
• Strong communication and clinical leadership skills.
• Ability to work in a fast-paced environment.

Responsabilités

• Serve as the facility’s primary internal medicine provider (SNF-ist).
• Manage care for short-term rehab and long-term residents.
• Perform comprehensive health assessments and diagnoses.
• Prescribe medications and treatments per protocols.
• Conduct monthly stability visits for long-term residents and coordinate with interdisciplinary teams.
