Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics-focused primary care experience to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers to deliver personalized, coordinated health care.

Qualifications

• APRN/NP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and plan care.
• Ensure compliance with regulatory agencies for clinical services.
• Prescribe medications and order labs and referrals.
• Monitor patient progress and adjust treatment plans.
• Collaborate with RN case managers as part of an integrated care team.
