Job summary

My Mobile MD LLC in Lafayette, IN is seeking a Nurse Practitioner or Physician Assistant to provide geriatric primary care in assisted living, independent living, and memory care communities across Lafayette, Lebanon, and Whitestown. The role offers a Monday–Friday schedule (8h/day) with on-site visits, strong clinical autonomy, and collaboration with a physician and an LPN on each team.

Qualifications

• Indiana NP or PA license (or eligibility)
• Board certification (AANP, ANCC, or NCCPA)
• 2+ years in primary care or geriatrics (NP/PA)
• Comfort traveling between communities with reliable transportation

Responsibilities

• Provide primary care and chronic disease management on site for geriatric residents
• Build lasting relationships with residents, families and community staff
• Collaborate with a full-time LPN in each community as case manager and support
• Practice with clinical autonomy under a collaborating physician
