Job summary

PruittHealth Premier Services seeks a Nurse Practitioner to collaborate with physicians and the care team to deliver high-touch, compassionate care to residents. Responsibilities include comprehensive adult/geriatric exams, disease management, and care coordination within a collaborative framework.

On-call pay is offered and there is no required evening or weekend onsite work. The role emphasizes quality, compliance, and seamless transitions across levels of care.

Qualifications

• Graduate of an accredited Adult or Family NP Master’s Degree program or PA Studies.
• Active RN/NP or PA licensure; board certification by AANP/ANCC (Adult/Family/Geriatrics) and CPR.
• Active DEA license and NPI/Medicare provider numbers; eligible for Medicare participation.

Responsibilities

• Perform comprehensive adult/geriatric admission history and physical exams.
• Identify, diagnose and document episodic, acute and chronic disease in adults.
• Reconcile medications and manage complex patients.
• Collaborate with physicians, case managers, social workers, and families to coordinate care.
• Participate in QAPI activities and ensure regulatory documentation in EMR.
