Job summary

PruittHealth Premier Services in Georgia seeks a Nurse Practitioner or Physician Assistant to join our long-term care health plan team. You will collaborate with physicians, RNs, and care staff to deliver high-quality, coordinated care and manage transitions between settings.

Required are NP or PA credentials with GA licensure, board certification, active NPI, and at least 2 years in geriatric/adult care. On-call pay and strong teamwork are key highlights of this role.

Qualifications

• Graduate of an accredited NP or PA program with state licensure.
• Board certification in AANP/ANCC for NP or PA credentials.
• Current CPR certification and active NPI/Medicare provider number.
• 2+ years geriatric/adult/family practice experience preferred.

Responsibilities

• Provide high-touch care in collaboration with physicians and staff.
• Perform comprehensive adult/geriatric assessments and care plans.
• Coordinate transitions across care settings and document in EMR.
• Reconcile medications and monitor chronic conditions.
• Participate in QA/QI activities and on-call duties.
