Job summary

TruHealth is seeking an advanced practice provider (NP/PA) to care for seniors in long-term care. You’ll work with a team of providers and case managers to improve outcomes for residents enrolled in Medicare Advantage plans.

The role emphasizes independent clinical judgment within nursing homes or assisted living settings. You will collaborate with facility partners and plan leadership to implement a national model of care, while ensuring compliance with federal and state regulations.

Qualifications

• FNP/AGNP/AHACNP or PA license required.
• Degree from accredited APRN/PA program.
• 3 years’ geriatric clinical nursing or rehab experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications (Word/Outlook/Excel).

Responsibilities

• Assess patients’ medical and mental health needs; develop plan of care.
• Ensure compliance with local, state, and federal agencies.
• Prescribe medications and order labs and diagnostics.
• Monitor patient response and modify treatment plans.
• Collaborate with RN case managers as part of an integrated care team.
