Job summary

TruHealth in Stanwood, IA is seeking an advanced practice provider (NP/PA) for primary care/geriatric care. You will work with a team to deliver proactive, coordinated health care in nursing homes and assisted living facilities.

The role requires an APRN/NP or PA license and a minimum of 3 years in geriatric clinical nursing or rehab, with EHR proficiency and MS Office skills. A strong team and career growth are highlighted.

Qualifications

• APRN/NP or PA license required.
• Degree from APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• MS Office applications knowledge (Word/Outlook/Excel).

Responsibilities

• Assess residents’ medical and mental health needs and plan care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and referrals.
• Monitor treatment responses and adjust plans accordingly.
• Collaborate with RN case managers as part of the care team.
