Aspire Physical Recovery Center at Cahaba River in Vestavia Hills, AL seeks a Nurse Practitioner (NP) to provide primary geriatric care. You will work with an interdisciplinary team to create personalized care plans for residents, diagnose and manage illnesses, and advocate for residents’ well-being.

The role emphasizes compassionate, patient-centered care, strong communication, and proficient EHR documentation as you collaborate to deliver high-quality care in a skilled nursing setting.

Formación

• Active NP certification with geriatrics focus.
• Minimum 2 years of clinical experience in geriatric settings.
• Strong geriatric assessment, diagnosis and treatment skills.
• Excellent communication, collaboration and interpersonal abilities.
• Compassionate, patient-centered approach to care.
• Proficient in EHR documentation.

Responsabilidades

• Conduct comprehensive examinations and manage acute and chronic geriatric conditions.
• Develop and implement individualized geriatric care plans.
• Prescribe medications and order/interpret diagnostics within scope.
• Collaborate with physicians, nurses, social workers and therapists.
• Educate residents and families about conditions and options.
• Advocate for resident well-being and ensure appropriate care.
• Document care accurately in the electronic health record.
