Job summary

LanceSoft is seeking an experienced Nurse Practitioner or Physician to join a growing healthcare team delivering primary care services to residents in assisted living communities throughout the Albany region. This full-time role involves travel between Hoosick Falls, Cambridge, and Troy to provide consistent, patient-centered care.

The ideal candidate holds an active NY NP or MD/DO license, DEA registration, and Medicare/Medicaid enrollment.

Qualifications

• Active New York State NP or Physician License.
• Minimum 3,600 clinical practice hours required for NYS independent practice.
• DEA registration active.
• Geriatric primary care experience preferred.

Responsibilities

• Deliver comprehensive primary care services to assisted living residents.
• Assess, diagnose, treat, and monitor acute and chronic medical conditions.
• Develop individualized treatment plans and preventive care strategies.
• Collaborate with facility staff, caregivers, specialists, and families.
• Ensure documentation and regulatory compliance.
• Manage patient care in accordance with Medicare and Medicaid requirements.
