Job summary

LanceSoft is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) for home-based primary care in NY. Provide high-quality, patient-centered care to high-risk individuals in a value-based program from Bronx/Westchester or Brooklyn/Staten Island, NY.

Responsibilities include diagnosing, treating, and coordinating care with an interdisciplinary team, with after-hours call rotation and EMR documentation. Requires NY license, national certification, and DEA; 3+ years of clinical experience.

Qualifications

• NP or PA from an accredited program.
• Active New York NP/PA license, National Certification & DEA.
• 3+ years of clinical experience.
• Geriatrics, home-based primary care, or chronic disease management preferred.
• Spanish bilingual preferred.

Responsibilities

• Provide home-based primary care for high-risk, medically complex patients.
• Diagnose and manage acute & chronic conditions.
• Perform assessments, prescribe medications, and order/interpret labs.
• Develop personalized care plans and educate patients/caregivers.
• Collaborate with interdisciplinary care teams.
• Maintain accurate EMR documentation.
• Participate in rotating after-hours call.
