Job summary

MedStaff Group is seeking a Nurse Practitioner to provide primary care services within skilled nursing facilities throughout the Cortland, NY area. This role offers autonomy, flexible daily hours, and a Monday–Friday schedule with an option for a 4-day workweek.

Base compensation ranges from $130,000 to $150,000 per year, with quarterly RVU bonuses. We offer strong administrative and clinical support, health/dental/vision, malpractice coverage, and PTO.

Qualifications

• Active New York NP license.
• Board certification: FNP, AGNP, or ANP.
• Active Medicare/Medicaid Provider ID.
• Experience in primary care, geriatrics or long-term care.
• SNF experience is a plus.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide primary care services to nursing home residents.
• Conduct routine visits, follow-ups, and chronic disease management.
• Collaborate with nursing staff and facility leadership.
• Participate in CCM and RPM care management programs.
• Maintain accurate documentation in SigmaCare, PCC, or similar EMR systems.

Responsibilities
