Blue Ridge Care seeks a compassionate Nurse Practitioner to deliver primary care within the PACE program. Under Medical Director supervision, you will perform comprehensive assessments, manage ongoing care, and collaborate with the IDT to support frail elderly participants.

You will develop care plans, treat acute and chronic conditions, and educate participants and caregivers while ensuring high-quality, coordinated care across settings.

Formación

• Must be a licensed RN and NP in Virginia with current CPR certification.
• Minimum 2 years of experience in geriatric or frail populations.
• Experience in multidisciplinary settings preferred.

Responsabilidades

• Perform comprehensive history and physical assessments for new participants.
• Develop and implement individualized care plans with the IDT.
• Evaluate and treat acute and chronic conditions in frail elderly participants.
• Coordinate referrals to specialists and maintain medical records.
• Provide education to participants, caregivers, and staff.
• Participate in IDT meetings and contribute clinical input.
• Ensure compliance with regulations and infection control procedures.
