Job summary

Continental Springs in South Sioux City, Nebraska is seeking a licensed Physical Therapist Assistant to work under the supervision of a Physical Therapist in our skilled nursing facility, supporting rehabilitation and independence for residents. You will implement treatment plans, guide therapeutic exercises, document progress, and collaborate with nursing, OT, speech therapy, and social services.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current PTA license in the state of practice.
• Experience in a skilled nursing facility or geriatric setting preferred.
• Knowledge of rehabilitation techniques and safety protocols.
• Familiarity with Medicare guidelines and documentation requirements.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of a team.

Responsibilities

• Assist Physical Therapist in implementing treatment plans to improve residents’ functional mobility and independence.
• Provide therapeutic exercises, gait training, transfer training, balance activities, and modalities as directed.
• Monitor resident response to treatment and report progress or changes to the supervising Physical Therapist.
• Document treatment sessions, progress notes, and billing information in accordance with facility policies and regulatory requirements.
• Assist with patient evaluations as directed by the Physical Therapist.
• Educate residents, families, and caregivers on exercise programs and safety.
• Ensure compliance with Medicare Part A/B guidelines, PDPM requirements, and state/federal regulations.
• Maintain a safe and clean therapy environment.
• Collaborate with interdisciplinary team members including nursing, occupational therapy, speech therapy, and social services.
• Participate in care plan meetings and discharge planning as appropriate.
