Job summary

Adept Rehab and Nursing of Omaha invites a dedicated Physical Therapist to join our rehabilitation team in a full-time capacity. You will evaluate residents, develop individualized care plans, and provide hands-on therapy per physician orders to improve strength, mobility, and independence.

Responsibilities include documenting progress, coordinating with physicians and nursing staff, and supervising therapy aides. Nebraska PT licensure is required; LTC experience is preferred but not mandatory.

Qualifications

• Current and unrestricted Physical Therapist license in Nebraska.
• Graduate of an accredited physical therapy program.
• Long-term care or skilled nursing experience preferred but not required.
• Strong clinical assessment, communication, organizational, and documentation skills.
• Ability to work effectively as part of an interdisciplinary care team.
• Regular and dependable attendance is required.

Responsibilities

• Evaluate residents using appropriate physical therapy assessment techniques.
• Develop and implement individualized plans of care based on each resident’s needs and goals.
• Provide hands-on physical therapy services per physician orders and treatment plans.
• Review resident progress and revise treatment plans as appropriate.
• Educate residents and families on exercises, mobility, safety, and functional activities.
• Complete progress notes, discharge summaries, home programs, and required documentation.
• Coordinate care with physicians, nursing staff, and rehabilitation team members.
• Supervise Physical Therapist Assistants and rehabilitation aides.
• Maintain resident privacy and HIPAA compliance.
