Job summary

FOX Rehabilitation in Sioux City, IA is seeking licensed Physical Therapist Assistants to deliver in-home, one-on-one care under a supervising Physical Therapist. You'll guide patients through personalized exercises and functional activities, helping them regain strength, mobility, and confidence in their own homes.

Qualifications

• Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.
• Basic computer skills; proficient in Microsoft Office.

Responsibilities

• Deliver individualized treatment plans established by your supervising PT and adjust activities based on patient response.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
• Document visits accurately in FOX’s EMR system, per Clinical Documentation standards.
• Coordinate care with patients, POAs, and community referral sources when needed.
