Vitae Health Systems in Iowa seeks a Psychotherapist to provide mental health services to residents in a skilled nursing facility. The role involves assessment, diagnosis, and treatment to support well-being and adjustment to medical changes.

You will collaborate with a multidisciplinary team, maintain documentation, and participate in care planning meetings while upholding HIPAA and ethical standards.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in the respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist).
• 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings.

Responsibilities

• Conduct psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, grief, cognitive decline, and adjustment to illness or disability.
• Collaborate with the Vitae clinical team, attending physicians, facility staff, social workers, and rehabilitation teams to coordinate care.
• Maintain accurate and timely clinical documentation compliant with regulations.
• Provide crisis intervention and behavioral management strategies when needed.
