Job summary

Eventus WholeHealth seeks licensed mental health professionals to deliver recovery-oriented rehab services in group or individual settings within skilled nursing and assisted living facilities.

Qualifications

• Master’s degree in social work and current license to practice in the state.
• Licensure as LCSW/LISW/LPCC/LMHC/LMFT or equivalent required.
• Knowledge of DSM and psychotherapy modalities.
• Ability to work in long-term care settings with adults in facilities.

Responsibilities

• Assess residents with consent and develop individualized treatment plans.
• Consult with primary care physicians, families, facility staff, and Eventus support staff.
• Document treatment plans and psychotherapy sessions within 48 hours for medical records.
• Coordinate with facility staff to review care needs and communicate findings.
• Maintain patient confidentiality and HIPAA compliance; adhere to laws and policies.
