Job summary

Eventus WholeHealth is seeking licensed clinicians (LCSW, LISW, LPCC, LCMHC or LMFT) to provide recovery-oriented rehabilitation in group or individual settings for residents in skilled nursing and assisted living facilities. You will work as part of a multidisciplinary team to deliver compassionate, evidence-based care that improves quality of life for residents.

Qualifications

• Master's degree in social work and an active state license is required.
• Knowledge of DSM and diagnostic tools.

Responsibilities

• Assess each resident to determine needs and develop a treatment plan.
• Consult with primary care physicians, families, and staff as needed.
• Document treatment plans and psychotherapy sessions within 48 hours for medical records.
