Job summary

Vitae Health is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ well-being and adjustment to medical life changes.

You will conduct psychosocial assessments, develop treatment plans, provide individual and group therapy, and collaborate with an interdisciplinary team while maintaining compliant documentation.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted license in-state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist).
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Experience in geriatric or long-term care settings preferred.

Responsibilities

• Conduct psychosocial assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Collaborate with interdisciplinary teams to coordinate care.
• Maintain accurate documentation in compliance with regulations.
