Job summary

Vitae Health Systems is seeking a qualified Psychotherapist to deliver mental health services to residents in a skilled nursing facility. You will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ well-being during medical life changes.

The role involves psychosocial assessments, treatment planning, individual/group therapy, and collaboration with the interdisciplinary team to coordinate care while maintaining HIPAA and regulatory compliance.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field.

Responsibilities

• Conduct comprehensive psychosocial assessments for residents upon admission and as needed
• Develop, implement, and monitor individualized treatment plans
• Provide individual, group, and/or family therapy sessions
• Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability
• Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns
• Collaborate with the Vitae clinical team, the patient’s attending physician, facility nursing staff, social workers, and rehabilitation teams to coordinate care
• Participate in care plan meetings and interdisciplinary team discussions
• Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations
• Provide crisis intervention and behavioral management strategies when needed
• Educate staff and families on mental health conditions and effective communication strategies
• Ensure compliance with HIPAA and ethical standards of practice
