Job summary

Vitae Health Systems is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents' well-being and adjustment to medical and life changes.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field.
• Active and unrestricted state license (LCSW, LPC, LMFT, or licensed psychologist).
• Minimum 2 years of clinical experience, ideally in geriatric or long-term care.
• Proficiency in evidence-based therapies (CBT, trauma-informed care, grief counseling).
• Strong assessment, diagnostic, and treatment planning skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct psychosocial assessments for residents at admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and family therapy sessions.
• Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability.
• Collaborate with the Vitae clinical team, attending physicians, facility staff, social workers, and rehabilitation teams.
• Maintain accurate and timely clinical documentation in compliance with regulations.
• Provide crisis intervention and behavioral management strategies when needed.
• Educate staff and families on mental health conditions and effective communication strategies.
• Ensure HIPAA compliance and ethics in practice.
