Job summary

Attain Behavioral Health is seeking a Group Facilitator – TBS & IOP in a nursing home setting to lead Therapeutic Behavioral Services (TBS) groups for older adults in Euclid, Ohio. The role blends mental health support, engagement, and relationship-building with residents who benefit from structured programming.

You will collaborate with facility staff, adapt content for varied cognitive needs, and ensure timely documentation while maintaining a compassionate, respectful environment.

Qualifications

• Must hold QMHS or QMHS III (REQUIRED).
• Experience facilitating behavioral health or substance-use groups.
• Experience working with older adults, long-term care, or medically complex populations preferred.
• Strong communication skills, empathy, and an ability to connect with residents patiently and respectfully.
• Able to work independently in off-site facilities.
• Reliable transportation required; Ohio Medicaid and background check requirements.

Responsibilities

• Facilitate meaningful TBS groups that support emotional health, coping skills, motivation, and quality of life for nursing home residents.
• Adapt group content to meet cognitive and emotional needs of older adults, including those with medical complexities or mild cognitive impairment.
• Collaborate closely with facility staff and on-site clinical team to ensure residents receive consistent, compassionate care.
• Complete documentation (group notes, progress updates, Medicaid paperwork) accurately and on time.
• Support resident engagement by encouraging participation, fostering connection, and creating a safe, respectful group environment.
• Participate in ongoing supervision and professional development as part of a supportive behavioral health team.
