UC Irvine's Division of Geriatric Medicine and Gerontology seeks an enthusiastic geriatrician for a full-time faculty position at the Health Sciences Clinical Series level (Assistant/Associate/Professor). The role focuses on innovative transitional care, linking hospital stays with skilled nursing facilities to help patients return to their communities.

The academic clinician will collaborate with UCI Health case management and SNFs, with a possible Medical Directorship and a strong emphasis on

Qualifications

• MD, or D.O from an accredited institution with appropriate clinical training and experience.
• Board certification in Family Medicine or Internal Medicine and board eligibility/board certification in Geriatric Medicine.
• Excellent clinical skills; interest in clinical teaching.
• Eligibility for UC Irvine Medical Center medical staff privileges required.
• Candidate must have demonstrated capabilities for teaching, professional services and/or research suitable to the HS Clinical Professor series.

Responsibilities

• Drive innovation in transitional care by bridging inpatient stays and skilled nursing facilities to return patients to their communities.
• Collaborate with case management teams at UCI Health and partner SNFs to streamline care pathways; Medical Directorship is possible.
• Contribute to teaching, quality improvement initiatives, and outcomes research within the division.

Education

MD or DO

Board certification in Family Medicine or Internal Medicine

Board eligibility/ certification in Geriatric Medicine

Excellent clinical skills; interest in teaching
