Job summary

PACE Southeast Michigan seeks a full-time Medical Doctor based on-site in Dearborn Heights, MI. You will provide primary and geriatric care to participants, including comprehensive assessments, diagnosis and ongoing management of acute and chronic conditions.

Work with an interdisciplinary team to coordinate individualized care plans, review updates, and adjust interventions to support health and independence. Documentation in EHRs and attendance at care conferences are expected.

Qualifications

• MD or DO from an accredited medical school.
• Residency in Internal Medicine, Family Medicine, or Geriatrics.
• Active, unrestricted Michigan license and DEA registration, or eligible to obtain.

Responsibilities

• Provide primary and geriatric care to PACE participants.
• Conduct comprehensive assessments and develop treatment plans.
• Collaborate with nursing, social work, therapy, pharmacy and others to coordinate care.
• Document in electronic health records and participate in care conferences.
