Job summary

Pace Central Michigan is seeking a physician to join our community-based team at Mt. Pleasant or Houghton Lake, focusing on geriatric and primary care for adults with disabilities.

You will collaborate with an interdisciplinary team to develop comprehensive care plans and ensure smooth coordination of services. The role emphasizes preventive care, care for chronic conditions, and patient education, with on-site scheduling and travel to affiliated sites as needed.

Qualifications

• MD/DO with Doctorate in Medicine and active license.
• NPI and CPR/AED certification required.
• DEA registration required.
• Michigan driver's license required.
• Board certification in Internal Medicine or Family Practice or board eligible; geriatrics certification preferred.
• Minimum of 1 year primary care experience with elderly/at-risk populations.

Responsibilities

• Member of interdisciplinary team; develops care plan and monitors health status.
• Supervising physician for nurse practitioner/clinical nurse specialist.
• Provides preventive, episodic care and health promotion; arranges referrals and conducts exams.
• Assesses testing, interprets screenings; plans follow-up; coordinates care.
• Educates members and families on health maintenance.
• Travel to Mt. Pleasant and Houghton Lake sites as needed.

Education

Doctorate degree in Medicine

NPI and CPR & AED

DEA registration

MI Driver's License

Board Certification in Internal Medicine or Family Practice

Advanced geriatrics certification preferred
