PACE Central Michigan is looking for a full-time Physician to join our Mt. Pleasant or Houghton Lake site. This role involves collaborating with an interdisciplinary team to provide comprehensive care for elderly individuals and adults with disabilities.

Qualifications

• Minimum of 1 year recent primary care experience with at-risk or elderly population.
• Board Certification in Internal Medicine or Family Practice or Board Eligible.
• Advanced certification in Geriatrics preferred.

Responsibilities

• Functions as a member of the interdisciplinary team to develop care plans.
• Provides leadership and participates in health status monitoring.
• Performs physical and mental status evaluations.
• Maintains documentation of health care services.
• Educates members and families regarding health maintenance.
