Job summary

Deer Oaks Behavioral Health is seeking a Licensed Professional Counselor to join our integrated clinical team in local long-term care facilities. You will deliver therapy, assessments, care planning, and behavioral health support on-site, with professional autonomy and a supportive administrative and clinical framework.

The role offers a Monday–Friday schedule, no evenings or on-call, robust resources, and opportunities for supervision, mentorship, and geriatric-focused training.

Qualifications

• Active, unrestricted LPC license
• Valid driver’s license
• Strong written, verbal, and interpersonal communication skills
• Ability to work independently while collaborating with an interdisciplinary team
• Comfortable navigating LTC environments
• Commitment to timely documentation and high-quality care

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Collaborate with nursing, social services, rehabilitation, and medical staff to coordinate care.
• Maintain documentation using the GNT and follow HIPAA guidelines.
• Build strong relationships with facility staff and leadership.
