Deer Oaks Behavioral Health is seeking a Licensed Professional Counselor to join our integrated clinical team, working onsite in long-term care facilities within your local community. You will provide therapy, assessments, care planning, and behavioral health support while enjoying professional autonomy and a predictable Monday–Friday schedule.

Qualifikationen

• Active, unrestricted LPC license.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to timely documentation and high-quality care.

Aufgaben

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, and other LTC conditions.
• Deliver behavioral management and brief, goal-oriented interventions.
• Collaborate with nursing, social services, rehabilitation, and medical providers to coordinate care.
• Maintain complete and timely documentation using the GNT.
• Participate in care plan discussions and facility meetings.
• Build strong relationships with facility staff and leadership.
