FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care to older adults in Greater Carlton, MN, in private homes or senior living communities. The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes through our Geriatric House Calls™ model.

Clinicians may incorporate telehealth and other virtual care to improve continuity of care, support more patients, and extend access to services in a home-based and community setting.

Qualificações

• Active Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilidades

• Provide one-on-one physical therapy to older adults in homes and senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and functional outcomes.
• Document care in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists as part of an interdisciplinary team.
• Educate patients, caregivers, and staff to promote aging in place and safety.
• Provide telehealth/virtual care when appropriate to extend access.
