Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to provide in-home and community-based therapy across Irving, TX and surrounding areas. You will assess, plan, and deliver evidence-based therapies to older adults in private residences or senior living communities, helping patients regain mobility and independence.

Qualifications

• Licensed Physical Therapist in the state(s) of practice, or eligible to obtain licensure.
• Degree from an accredited Physical Therapy program.
• Basic computer/EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations and develop individualized care plans.
• Deliver therapy sessions to improve mobility, independence, and function.
• Document in the EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate.
