Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist for in-home and senior living care in Spring, Texas. You will deliver one-on-one therapy to older adults in residences or communities, assess and plan care, and document in FOX's EMR system, ensuring regulatory compliance.

You will collaborate with an interdisciplinary team and may incorporate telehealth to enhance access and flexibility, while shaping patient outcomes and independence in daily living.

Qualifications

• Must hold a valid Physical Therapy license in state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Current CPR certification and basic computer/EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, independent living, assisted living, memory care).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX's EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
