Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist Assistant (PTA) for PRN home visits in St. Augustine, FL. The role focuses on geriatric therapy to help residents maintain independence and age in place.

You'll provide skilled PT treatments under a plan of care, document accurately, and collaborate with physicians and families. Requirements include an Associate degree, active Florida PTA license, and flexibility for an as-needed schedule.

Qualifications

• Associate degree from an accredited PTA program.
• Active Florida PTA license.
• Two years of clinical experience preferred (not required).

Responsibilities

• Provide skilled physical therapy treatments under the plan of care.
• Help patients improve mobility, function, and independence.
• Communicate effectively with physicians, families, and the care team.
• Complete accurate, timely documentation.
