Job summary

Monroe County Medical Society is seeking a Nurse Practitioner or Physician Assistant to lead the Physician House Calls (PHC) program. You will provide comprehensive primary care to homebound older adults in their own homes, under medical supervision.

The role emphasizes independence, collaboration with an interdisciplinary team, and a commitment to minimizing hospitalizations through person-centered, home-based care. NY licensure and geriatrics/primary care experience are preferred.

Qualifications

• Nurse Practitioner with a master's or doctoral degree in geriatrics, family, or adult primary care.
• Physician Assistant with master's degree and relevant experience, licensed in New York State.

Responsibilities

• Provide comprehensive primary medical care for homebound older adults in their residence.
• Collaborate with interdisciplinary team and program leadership to coordinate care.
• Promote independence and reduce emergency visits through person-centered home-based care.
