Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one geriatric care in Cresskill, NJ. You’ll evaluate, plan care, and provide evidence-based therapy in homes or senior living communities, with optional telehealth.

You’ll enjoy autonomy, flexible scheduling, and professional growth within a clinician-led environment. Applicants must hold a NJ PT license and CPR certification.

Qualifications

• Active Physical Therapy license in NJ or eligibility.
• Graduate of an accredited PT program.
• Current CPR certification.
• Ability to document effectively in an EMR system.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver sessions that improve mobility, independence, and functional ability.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
