Job summary

FOX Rehabilitation in Rumson, NJ seeks licensed Physical Therapists who want to provide one‑on‑one care to older adults at home or in senior living communities. This role supports patients in improving mobility, independence, and quality of life through our Geriatric House Calls™ model.

You will evaluate, design plans of care, and deliver evidence‑based therapy, collaborating with PTs, OTs, SLPs and staff, with options for telehealth to extend access.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences, senior living communities, independent living, assisted living, and memory care settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate to extend access and flexibility.
