Job summary

WellBe Senior Medical seeks a Physician Assistant for adult home health in Charleston, WV. The role centers on geriatric, chronic, and acute care delivered in patients’ homes, coordinating with a physician-led interdisciplinary team to ensure comprehensive, concierge-level care across the care continuum.

Qualifications

• Master’s degree in Physician Assistant Studies required (ARC-PA)
• 3–5 years PA experience (not open to new grads)
• Active WV PA license and NCCPA certification required

Responsibilities

• Perform in-home visits to support patient care needs
• Coordinate with care community to manage health and wellbeing of homebound patients
• Order, interpret, and follow up on diagnostic tests and treatments
• Prescribe and manage medications per state regulations
• Maintain up-to-date patient records and document care plans
• Deliver palliative and end-of-life care as needed
