Job summary

Halcyon Home is a fast-growing home health, hospice, and home care company serving the Central Texas area. We are hiring a Full Time Home Health Occupational Therapist to support our geriatric population in Kyle/Buda and surrounding areas.

You will perform clinical assessments, develop and evaluate care plans, educate patients and caregivers, and coordinate with physicians. Travel locally is required, with ongoing education and team support.

Qualifications

• Bachelor's degree required in Occupational Therapy and active state license.
• Minimum one year of home health experience preferred.
• CPR certification and strong interpersonal skills required.

Responsibilities

• Perform ongoing clinical assessments to determine patient needs.
• Evaluate patients’ occupational performance in self-care, work, leisure, and rest.
• Assess and facilitate therapeutic interventions for ROM, strength, coordination, adaptive equipment, and functional mobility.
• Develop and evaluate patient care plans in collaboration with patients, caregivers, and representatives.
• Document clinical notes, phone orders, and other records promptly and accurately.
• Provide education and counseling to patients and caregivers.
• Supervise and guide occupational therapy assistants and home health aides as necessary.
• Apply infection control and standard precautions during patient care activities.
• Communicate effectively with physicians and healthcare practitioners involved in the patient’s care.
• Meet continuing education requirements and demonstrate professional growth and competency.
• Promote the Agency’s philosophy and policies to ensure high-quality care.
